Can anyone tell me what type of shaft seal this is?
There isa grease nipple on the top of it, i presume more grease should stop or slow the drip which is about 1 drip every six seconds at the moment. There doesn't seem like there's any way to tighten it. (You can see where the water got to)
Also the rubber? hose needs replacing, where do i get the proper hose from?

It looks like a normal packing gland but with a different adjustment arrangement. Are you sure the end near the shaft doesn't turn? Is it knurled? It should be obvious how it works when you get it out.
Get the hose from any marina workshop.
It is looking very sad, I wouldn't delay pulling it out and giving it a total service.
Or, you could do what I did and replace it with a Volvo gland. The occasional grease is all it needs and the bilge is dry. No drips, ever.

I don't think the shaft end of it is knurled, it looks like carbon??
Where do you get the Volvo glands, a search has come up with nothing?

My vote for the Volvo stern gland too. I've always fitted them to my boats with no problems. The major French manufacturers put them on all their boats with tens of thousands of them working well.
bluemoon you may need to replace your shaft too if there's any corrosion or pitting on it . I had to when I fitted a Volvo seal to the old h28. the shaft looked good but once I got it out it was terrible.

my guess is its a lip seal, the gland will have 2 lip seals at least, one either side of the grease nipple. You may/should find some sort of circlip at the front face of the gland. The front face of the seal will have its size on it.
I replaced mine early this year. Looks very similar. I did it with the boat in the water. You can do it with a bit of planing, its no big issue. Your heart skips a beat when you pull the gland out and water starts to flow into yourboat but thats where planing comes into it!!
